Create a Tandem Source Account

Note: Creating an account requires access to your email to confirm account creation.

2. Select Create Account (under Log In button)

3. Select type of account, Personal Use or Parent/Guardian Use

4. Fill out Patient Information

5. Check email to Activate your Account

6. Finish creating account by choosing Password
